Evaluation Form
This form is designed to help us evaluate our SNAP plan and to ensure that we are working towards our SNAP objectives. It is essentially up to you on how often you would like to record this data. All that is required is that you use this form and that you are prepared to report your data at each CNPC meeting (approximately every 6 months). An email will be sent out every three months to remind you to keep track of how you are using the SNAP materials in your area.
